Collaborative Care & Referrals
We communicate with primary care providers, specialists, and therapists (with permission) to support
coordinated treatment plans and smooth handoffs between in-person and virtual care.
Follow-Up After In-Person Visits
Some patients begin care in person and then transition to telehealth for follow-up visits when appropriate.
Your provider will discuss what is safest and most effective for you.
When In-Person Care Is Needed
In certain situations, such as higher levels of risk or complex medical needs, in-person evaluation may be
recommended instead of telehealth. Your provider will discuss options and next steps.
